A new context and vision for immunizations
• Introduction of new vaccines, new compositions, new presentations, and new indications
• Future vaccines
• Diversity of vaccine providers
• New availability of funds
• Global and Regional recommendations
But…
• Multiple health priorities, limited human resources and logistical capacities
• Expensive vaccines with limited funds available
• An expressed need for national adaptation of global and regional recommendations to take into consideration local realities
• Existence of immunizations and vaccines outside of EPI: Absence of recommendations for some groups such as persons consulting the private health sector, military personnel, workers, tourists, school children, adolescents...
• Limited mandate of Inter-agency Coordinating Committees (ICCs)
• Some ICCs have technical sub-committees but may focus only on EPI or work primarily on operational coordination
A request from countries
• Based on successful existing national independent committees (some established with support from WHO Regional Offices), such as those in Thailand, Brazil, China, Sri Lanka, USA, Canada, Australia, South Africa, UK, France and other locations.
• World Health Assembly 2008 (WHA 61.15): "WHA requests the Director General to [...] strengthen national capacity for making evidence-based policy decisions to adopt new vaccines"

• WHO Strategic Advisory Group of Experts (SAGE) meeting (WER, No. 1-2, 9 January 2009)
- One of WHO's priorities as part of the process of ensuring evidence-based decision-making at country level is to establish or strengthen national immunization technical advisory committees, which are increasingly called upon given the complexity of immunization programs and the high cost of new vaccines.
